Here are the forms you'll need as part of the yearly computer deployment process.

 

The Laptop Computer Usage Contract is required for a student in grades 6-12 to sign out a computer. Students in PK-5 should also have a Usage Contract on file as they will use a chromebook in their classroom.

 

The Technology Protection Plan is optional. 

 

These forms are also part of the Online Registration (or Verification) Process.
